You'll need two forms of identification listed below:

  • Driver's License
  • Social Security Card
  • ITIN Card
  • Birth Certificate
  • Passport

Note: If the address on the ID does not match the address the member provides, an additional document may be required to verify the address (such as a lease agreement, utility bill, or similar document).
